Remote Patient Monitoring

Intersect provides a range of services designed to assure your patients meet their treatment goals and are successful in their remote monitoring program. We do this by taking steps to reduce the tasks they need to accomplish to get started, and to asure they understand how to use the mobile and medical devices to complete their scheduled testing and that help if available if needed. Intersect also offers system orientations to providers and medical staff members to assure they are able to utilize the Intersect on FHIR web application.

Intersect achieves patient-related tasks through an established process. Steps in the process are:

  • Provision the mobile device by equipping it with a cellular mobile SIM chip and enrolling it in the Esper device management service which, in addition to enabling remote troubleshooting access, restricts the device access to only the Intersect mobile application ; install the Intersect mobile application; activate bluetooth and assuring a connection to the patient's prescribed medical devices.
  • Shipping the configured tablet to either the patient or provider's office for delivery to the patient
  • Scheduling and delivering a remote introduction and device orientation to assure patient can navigate and use the devices
  • Staffing an extended business hours helpline to assist patients and providers with usage issues
  • Remote device access to troubleshoot and resolve problems and to make application version upgrades

Our web application, Intersect on FHIR, clearly displays patient test information and trends so that trends and anomolies are visible without a requirement to search for them. At a glance, aided by alerts, providers and medical staff can determine when, between scheduled virtual visits, remedial action is required.

Patient care is delivered by your providers and medical staff. Dependent on the type of remote monitoring program patients are enrolled in, a majority of the monitoring activities can be delivered by medical staff under the supervision of the patient's provider. A limited number of virtual physician encounters may be required. Medical staff can be:

  • Certified Nurse Assistant (CNA)
  • Certified Medical Assistant
  • Certified Nurse Specialist (CNS)
  • Nurse Practitioners (NP)
  • Physician Assistant (PA)

The elements described above were incorporated into Intersect's service offering because they are determined to be necessary to insure patient and provider satisfaction with a remote monitoring initiative of the scope required for patients with complex healthcare conditions. They also assume that Intersect can more efficiently deliver the services than could be done by healthcare organizations leaving them to focus on treating their patients.
