In 2018, according to the CDC, 51.8% of patients had multiple chronic conditions. A study at Northwestern Medicine with a group of asthma patients in 2022 revealed that out of 77 asthma exacerbation encounters, the median length of time between exacerbations for patients who saw their outpatient provider within one month was 131 days whereas the median length of time between exacerbations for patients who did NOT see their outpatient provider within one month was 46 days. The case for remotely monitoring and managing those patients is compelling. The potential exists to further reduce the readmission rate for all of them at a much lower cost.
