When a physician opens his/her Intersect on FHIR dashboard, they can see at a glance their patients, patient's last screening results, their virtual appointments, patient messages, and personal reminders. To view a patient record, all that is required is a double click on the patient name. The patient record is opened displaying the patient's history with current medications and allergies and adverse reactions populated from the on-premises EHR. Click on a button to initiate a new virtual encounter. Later when the encounter is complete, the information that was created during the virtual visit, along with the remote monitoring tests, can be viewed from the on-premises EHR.

In 2018, according to the CDC, 51.8% of patients had multiple chronic conditions. Many of those patients live in a continuous cycle that ranges from near-normal functionality to emergency department visits and in-patient admissions and back again. The costs to patients and healthcare organizations is a staggering amount of the total healthcare spend. Patients with multiple chronic disease commonly make frequent visits to the Emergency Department and have a high occurrence of expensive in-patient hospital readmissions consuming thousands of dollars of unreimbursed services. Frequent ED users count for more than 60% of the total visit volume. The average readmission cost was $15,200.

A reduction of either of those occurrences can result in a significant savings. A study at Northwestern Medicine with a group of asthma patients in 2022 revealed that out of 77 asthma exacerbation encounters, the median length of time between exacerbations for patients who saw their outpatient provider within one month was 131 days whereas the median length of time between exacerbations for patients who did NOT see their outpatient provider within one month was 46 days. The case for remotely monitoring and managing those patients is compelling. The potential exists to further reduce the readmission rate for all of them at a much lower cost.

Prevention of chronic disease for patients at high risk and tight control of those already afflicted will have a marked improvement of their quality of life, while creating a new revenue stream for the organization and reducing costs from readmissions and frequent ED visits.
